On pp 342-3 of 'English Professional Theatre' the excerpted text of John Hyde's testimony of 8 December 1590 is given under the date 12 February 1592, his testimony of 12 February 1592 is under the date 21 February 1592, and his testimony of 21 February 1592 is under the date 8 December 1590. There is a corresponding confusion of shelfmarks and page references to Wallace's 'First London Theatre'.
